Online Initial Consultation

Nutrition Consultation Summary

An online consultation is completed via phone or video call. Please note that online consultations are only available to individuals living within New Zealand only. On booking an online nutrition consultation I will email a detailed nutrition and lifestyle questionnaire including a 3-day food, exercise and sleep diary. This needs to be completed and returned at least 24 hours prior to your appointment to ensure you gain maximum benefit from your consultation. The information you provide will help me assess your current issues, dietary intake, and understand your lifestyle. During the consultation we will discuss the information provided on your pre consultation questionnaire, your health concerns and goals. Together we will create simple sustainable recommendations tailored to your nutrition and lifestyle goals. After the consultation, you will receive via email the recommendations discussed as well as any relevant resources.

Cancellation Policy

I understand that sometimes it is necessary for you to change or cancel your appointment. Please provide a minimum of 24 hours' notice if you do wish to change or cancel your appointment. If insufficient notice is given, or you don't turn up for your appointment, a cancellation fee of $30 will be charged for initial and follow-up appointments. There will be no refunds given for participants who do not turn up to group nutrition sessions or if insufficient notice is given.
